Blackburn Center works with providers, medical staff and community partners to help people experiencing or at risk of homelessness stabilize and rebuild their lives. We specialize in serving those with complex needs associated with homelessness, including medical, substance use, mental health and housing issues.

You can refer patients to Blackburn Center for all, or just one, of our services:

  • Treatment for Hepatitis C

  • Medication supported recovery
  • Mental health treatment: medication, counseling, groups and case management

Recuperative Care at Blackburn center

Not all patients have a home to return to when discharged from the hospital. Through our Recuperative Care Program, we partner with the hospital system so that patients can be discharged directly to CCC housing. Patients are picked up from the hospital, given their own room and are immediately established with a primary care provider and case management team. Case managers check in daily with their clients, take them to follow-up appointments and assist them with their daily needs. Additional addiction and mental health care services are also available on-site at Blackburn Center.
